AllCare.ai delivers coordinated, in-facility care (Primary Care, Psychiatry, Podiatry, Diagnostics, Wound Care, CCM) with a 24/7 concierge. Our platform automates charting and compliance so you can focus on high-quality senior care.

Role Overview
We're seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.

What You'll Do

  • Routine & urgent assessments; chronic disease management
  • Post-discharge follow-ups, med reconciliation, labs/diagnostics
  • Coordinate with facility staff, specialists, families/POAs
  • Same-day documentation; CMS/regulatory compliance

Schedule & Setting
Part-time position with a structured weekday schedule. You'll spend 3 days per week delivering care, with some days on-site across Stockton, Modesto, and Amador (strategically clustered routes designed to minimize travel and maximize time with patients) and other days via telehealth.

Requirements

Qualifications

  • Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
  • Geriatrics or facility-based experience preferred; EMR proficiency
