Job description

Description

Care Management Assistant | Care Management Department | Springfield, Oregon
Part Time | 0.68 FTE | Day Shift (0800-1730)
Pay Range: 21.59 - 32.39 per hour

If you're looking for a role where every phone call, referral, appointment, and discharge plan helps a real person move safely to their next stage of care, this is your opportunity.

Many healthcare support roles can feel like a never-ending queue of tasks with little connection to the people behind them. At PeaceHealth, you'll be a key part of helping patients navigate one of the most important moments in their healthcare journey: getting home safely with the support they need to recover and thrive.

Why PeaceHealth? Why Now?

At PeaceHealth Sacred Heart Medical Center at RiverBend, you'll join a Care Management team that helps remove barriers for patients every day, ensuring they leave the hospital with the equipment, services, appointments, and support needed for a successful recovery.

What's in it for you?

  • See the impact of your work daily. You'll help coordinate home health services, follow-up care, and resources that directly affect patient outcomes.
  • Be more than an administrator. You'll partner closely with RN Care Managers and Social Workers to support meaningful transitions of care.
  • Build relationships, not just spreadsheets. You'll interact with patients, families, providers, clinics, and community partners throughout the discharge process.
  • Work in healthcare without losing the human connection. Every task you complete helps a patient take the next step safely and confidently.
  • Join a mission-driven organization. PeaceHealth is rooted in compassion, respect, and serving the communities of the Pacific Northwest.

What You'll Do

You'll be an important part of helping patients move from hospital care to the next stage of recovery.

In this role, you will:

  • Help arrange home health services and medical equipment needed after discharge.
  • Coordinate follow-up appointments with primary care providers and specialists.
  • Assist patients who do not currently have a primary care provider in establishing care.
  • Support arrangements for outpatient therapy and other post-hospital services.
  • Provide discharge planning support in partnership with RN Care Managers and Social Workers.
  • Document care coordination activities accurately in the electronic medical record.
  • Deliver Medicare Important Message letters and verify patient demographic information.
  • Communicate with clinics, providers, insurers, and community partners to support patient care transitions.
  • Welcome visitors, answer phone calls, and connect people to the appropriate team members.
  • Support department operations through scheduling, reporting, data entry, and other administrative tasks.
  • Help ensure patients, families, and care teams receive timely information and support.

What You'll Bring

Required

  • High School Diploma, GED, or equivalent.
  • Three years of administrative experience in a professional office setting.
  • Strong communication and customer service skills.
  • Excellent organizational skills and attention to detail.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Proficiency with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint.

Preferred

  • Experience in a medical office, healthcare setting, or hospital environment.
  • Administrative assistant or business support training.
  • Familiarity with electronic medical records and healthcare terminology.

Nice to Have
