Job description

As a Day Center CNA for Swope Health Services, you will be part of a team of caregivers to provide care for the people who need it most. The Day Center CNA provides quality care to participants, under the supervision of the Supervisor of the Day Center and nurses in accordance with PACE KC policies and procedures. You will also perform related tasks in accordance with the participant's plan of care. If you have a passion for working with a team of dedicated caregivers to make care visible every day, this role is for you.
Primary Responsibilities:
  • Consults daily with nurses and providers in managing participant flow. Assists in prioritizing the scheduling of participants.
  • Schedules necessary appointments for participants for clinic, specialists, and outside medical appointments as requested.
  • Takes vital signs of participants, escorts participants to exam rooms, and prepares participants to be seen in the clinic.
  • Reports abnormal findings and changes to the nurse and/or primary provider.
  • Assists participants in disrobing, positions participant in bed, chair, or exam table and ensures they are ready to see nurses and physicians as appropriate.
  • Assists in any medical procedures as requested within scope of practice and competency.
  • Maintains up-to-date and appropriate documentation in the participant's electronic medical record.
  • Assists in maintaining cleanliness of exam rooms and necessary supplies/materials are stocked as needed.
  • Responsible for ensuring all lab specimens are labeled and ready for courier to pick up in a timely manner.
  • Prepares list of medical supplies to be ordered by the clinic nurse.
  • Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
  • This position may require you to provide care at participant's residence as needed.
  • Participates in in-service training and staff meetings as required.

Successful Candidates will have:
  • Current Certified Nursing Assistant (CNA) Certification
  • 1 year of experience working with the geriatric population.
  • Knowledge of principles, practices, standards, and techniques of a CNA.
  • Ability to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care.
  • Proficient with Electronic Medical Records (EMS)
