Job description

Contracting Nursing Home RN

The Contracting Nursing Home RN (CNH RN) demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication, and coordination of resources. Use principles of relationship-based care while providing oversight of care provided by the contracted nursing homes. Works with the interdisciplinary team, community agencies, and patients/families using Relationship Based Care principles. Assesses patients in contracted community nursing homes and adult day care facilities. Completes contract nursing home inspections with the interdisciplinary team, and coordinates discharges from acute care settings by reviewing supporting documents for skilled care and other patient care needs in a timely manner to assist in determining the appropriate level of care for the residents. A resource and liaison to the contracted nursing facilities for any other needs the residents may have. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care. Demonstrates performance and leadership that is broad enough to improve the care for a group of patients. Responsible for the documented outcomes at the program or service level. Demonstrates leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning. Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes. Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities. Systematically evaluating current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise, Guiding, developing, and supporting staff from a leadership perspective. Use professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities Assumes responsibility for the coordination of veterans residing in a VA-contracted Community Nursing Home/Adult Day Care Center focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.

Pay: Competitive salary, regular salary increases, potential for performance awards

Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Licensure: 1 full and unrestricted license from any US State or territory

Work Schedule: Monday - Friday, 8:00am - 4:30pm
