Job Summary and Responsibilities
RN Hospice Home Visit Travel - Durango, Colorado

Job Requirements
The Registered Nurse, Travel provides high-quality, compassionate, and ethical nursing care to patients in their homes, across various locations nationally.  This position will work independently and collaboratively with the interdisciplinary team to deliver care to patients who are rehabilitating, managing chronic conditions, or are in the end stage of life.  This position requires a highly skilled and adaptable RN with strong clinical, communication, and organizational skills.
 
 
Qualifications
 
  1.   2. Clinical Care: Provides direct nursing care to patients in their homes, following established care plans and physician orders.  This includes, but is not limited to: assessment, medication administration, wound care, teaching, and emotional support.  Accurately documents all care provided in a timely and accurate manner.
  3. Compliance: Adheres to all applicable federal, state, and local regulations and guidelines, including HIPAA, OSHA, and infection control protocols. Maintains confidentiality of patient and employee information.
  4. Collaboration: Collaborates effectively with physicians, other healthcare professionals, patients, families, and caregivers.  Participates in care coordination meetings and communicates effectively regarding changes in patient condition.
  5. Case Management:  Manages a caseload of patients, meeting productivity expectations, prioritizing care needs and ensuring seamless transitions of care.  Proactively identifies and addresses potential issues or concerns.
  6. Documentation:  Completes all required documentation accurately and timely, including but not limited to: electronic health records (EHR), visit notes, care plans, and other required forms.
  7. Professional Development:  Maintains current knowledge and skills through continuing education and professional development activities.  Actively seeks opportunities to enhance clinical expertise.
  8. Travel:  Willing and able to travel extensively to various locations nationally, as assigned, with flexibility to adapt to different work environments and patient populations.  Responsible for all travel arrangements and expenses as outlined in the company's travel policy.
  9. Safety:  Maintains a safe work environment for self and patients, adhering to all safety protocols and reporting any safety concerns immediately.
  10. Professionalism:  Maintains a professional demeanor and appearance at all times, representing CommonSpirit Health at Home positively to patients, families, and colleagues.
