Job description

Position Overview: The Case Manager will provide professional clinical care to patients with behavioral healthcare needs and work with clinical and case management staff to coordinate care and assist with treatment and continuing care plans for patients.
Specific Responsibilities:
Completes and/or reviews Biopsychosocial Assessment, identify all life domain needs, and incorporate these identified needs into the Continued Care Plan to ensure patient has continued care for each identified need as assigned. Completes and/or supports the ongoing developments of the treatment plan working collaboratively with the multidisciplinary team, as well as supports patient's ongoing engagement in treatment as assigned. Completes consents to release information for all identified supports in patient's ecosystem (natural supports, referents, existing treating providers, PCP). Collaborates with the patient and patient's family or natural supports to implement plans for evolving continued care plan. Follows all referent protocols. Initiates and manages FMLA and Short-term Disability applications as needed, with patient consent. Coordinates with patients and their employers to facilitate benefit processes when applicable.
Communicates progress to multidisciplinary team and documents patient's progress in accordance to regulatory requirements that include but not limited to: documentation of activities performed as part of the service delivery process (e.g., assessments, treatment planning, coordination of care, discharge planning). Schedules all appropriate appointments for services identified in the patient's treatment plan to address area of needs, such as but are not limited to: substance use disorder, mental health disorder, MAT, medical, legal. Confirms all appointments on the continuing care plan prior to discharge and ensures all efforts were made to schedule these appointments within the patient's in-network providers. Carries out RCA Care Model with fidelity. Sends all Continued Care Plan documentation to receiving treatment providers. All other duties as assigned
Minimum Qualifications
Bachelor's Degree in related field is preferred. High School Diploma/GED/Equivalent and minimum of one (1) year of professional experience in Behavioral Health/Substance Use Treatment field required. Knowledge of health care, detoxification process, addiction, co-occurring disorders, DSM Criteria, and terminology. Ability to read and interpret written information; write clearly and informatively; edits work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings. Working knowledge of Microsoft Word, Excel, and Outlook.

Collaboration: Clinical leaders must be able to work in collaboration with other health professionals and leaders from other disciplines.
Work Environment: This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.
Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus.
Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
