Outreach Specialist and Health Coach

JOB SUMMARY:

The MHVT Outreach Specialist will address chronic disease prevention and management through the implementation of My Healthy VT (MHVT) Self-Management programs (SMP) and MHVT Health Coach Pilot Program. This includes working within their coverage area to increase awareness of and referral to MHVT programs using a localized approach, including electronic health record (EHR) panel management, local community and provider outreach, coordinating with BP QI facilitators, and connecting with specialty practices to ensure MHVT programming is offered to patients, particularly when specialty services have a wait list.

PRE-REQUISITES:

Education: Associate Degree, or High School degree with 3 years of relevant experience, and NBC-HWC health coach credential

Experience: Administrative and health coaching experience. At least two years working in health promotion, health education, health administration, nutrition, and/or self-management or other health promoting programs.

Other Skills: Strong written and verbal communication skills. Strong sense of organizational commitment, customer orientation, performance focus, and self-management. Proficiency in Microsoft Office applications such as Outlook, Excel, and Word required. Proficiency in Web meeting platforms such as Microsoft Teams. Grant management skills. Use tact, diplomacy, and professionalism at all times, Ability to work independently and meet stated goals. Commitment to excellent and culturally responsive customer service and communication. Proven ability to work collaboratively with individuals and groups.

SCOPE:

Machinery or Equipment Used: Personal transportation, laptop, computer, phone

Physical Demands: Ability to travel to multiple community sites in one day; ability to enter information into a computer, understanding the task will require sitting for extended periods of time; repetitive motions of the wrist, hand, and/or fingertips; stooping, kneeling, reaching, pushing, pulling, and lifting up to 10 pounds; sedentary work with occasional walking or standing; the employee is not substantially exposed to adverse environmental conditions

Working Conditions: Hospital and community settings. This position includes in-person visitation to various locations within the region- means of private transportation is required.

ESSENTIAL FUNCTIONS:

· Actively participate in the statewide MHVT meetings and collaboration

· Refer to MHVT programs as appropriate

· Provide targeted outreach to local community agencies, like Senior Centers, Agencies on Aging, congregate meal sites, libraries, pharmacies and health clubs

· Participate in local events and marketing opportunities, such as health fairs, community health events etc.

· Host MHVT workshops in accordance with training and availability

· Connect with patient-facing staff at health care practices and specialist practices in Franklin/Grand Isle/Lamoille Counties to ensure MHVT workshops and health coaching are offered to patients

· Work alongside physicians, nurses, dietitians, therapists, and other professionals to support patient or client behavior change

· Provide one-on-one health coaching to NMC patients and MHVT program participants

· Support and empower patients through goal setting, accountability, and culturally sensitive health coaching

· Offer health coaching to all patients with the goal of preventing, delaying or managing chronic conditions. When appropriate, referrals will be made to MHVT for workshops including Chronic Pain, Chronic Disease, Diabetes Prevention and Management, High Blood Pressure, and Quit Smoking

· Conduct pre-and post- MHVT Health Coach Pilot assessments with patients

· Commitment to cultural competence, consistently respecting and supporting people across cultures, races, ethnicities, bodies, abilities, genders, and identities.

· Demonstrate thorough understanding of social determinants of health
