Job description

Position Title: Hepatitis C Treatment Program Coordinator
Report to: Director of Operations
Position Goals and Role:
This is a full time position at REACH Medical
REACH Medical provides low-threshold, harm reduction care in a multi-disciplinary, integrated
practice to further the REACH Mission of ensuring health equity to all who typically face stigma
in the current healthcare system.
The HCV Program Coordinator serves as the lead coordinator of the elements of the REACH
hepatitis C program. This program aims to provide this service to REACH patients located
across New York State via telemedicine.
Core responsibilities include the development, implementation and maintenance of workflows to
ensure that REACH provides low threshold hepatitis C screening and treatment for patients
across NYS while needed metrics and data are preserved and reported as needed. The
HCVcoordinator will supervise the Hepatitis C Peer Navigator in their role to provide
programmatic education, intake, testing, treatment, patient navigation and the management of
the patient incentive program. All program activities will address and empower each
participant's efforts to reduce risk, screen and treat Hepatitis C.
Successful candidates will be certified as a phlebotomist; and or successfully coordinated
other health care related grant programs; and or have clinical knowledge around hepatitis
C; and will be available to drive and travel to surrounding counties. All candidates must
demonstrate a commitment to harm reduction theory and practice, commitment to advocacy,
particularly in the case of HIV/AIDS, hepatitis C, homelessness and drug policy.

Responsibilities:
Collaborate with the Chief Medical Officer (CMO) or designee to provide clinical
coordination of the hepatitis C program, demonstrating mastery of the most recent
guidelines from the American Association for the Study of Liver Diseases (AASLD)/
Infectious Diseases Society of America (IDSA) guidelines for the treatment of Hepatitis
C
Work collaboratively with clinical Hepatitis C experts at Buffalo University who serve as
a resource for REACH providers in the treatment of Hepatitis C

Serve as the point of contact for all clinical questions related to Hepatitis C relaying
questions as needed to the CMO (or designee) or clinical Hepatitis C experts at Buffalo
University
Work with pharmaceutical representatives and pharmacies to ensure that patients receive
appropriate Hepatitis C treatment
Oversee all required prior authorization processes for the entire duration of treatment.
Work collaboratively with the AIDS Institute Bureau of Hepatitis C Healthcare providing
accurate data, monthly narratives and attending Bureau meetings and trainings as
required.
Work collaboratively with the University of Albany's InSide Study team on managing
study requirements and incentives for patients. Provide accurate data and attend study
meetings and trainings as required.
Attend designated REACH meetings and keep accurate minutes/records when
necessary to ensure that the Peer Navigator and REACH team actions are aligned
with the goals of the study and the grant.
Work collaboratively with the operations team to develop, implement and navigate
workflows to ensure the provision of low threshold hepatitis C screening and treatment
Work collaboratively with other Hepatitis C navigation programs at syringe service
programs in NYS
Further develop and maintain a database of patient demographics, data metrics, patient
outcomes and the roll out of patient incentives effectively capturing them in reporting
platforms
Oversee the activities of the Hepatitis C Peer Navigator ensuring that patients are
enrolled, get labs drawn, receive incentives, attend appointments and receive and take
medication
Perform lab draws in house and in the community or facilitate with the Peer Navigator
transportation to laboratories where labs will be drawn
Travel to counties outside of Tompkins, based on the grant parameters and requirements
Coordinates labs, appointments, insurance, pharmacy, and supportive services.

Anticipates barriers like limited phone access, transportation, mistrust, and insurance
denials.
Uses flexible communication, bundled appointments, ride services, and close
collaboration with pharmacies.
Ensure the Initiation of treatment within 1 week of RNA confirmation.
Provides medication delivery and blister packs for unstable clients.
Escalates support during crises, with real-time client registry to prevent dropouts.
Coordination with our in house Nurse, and clinical team to time treatment appropriately
and supports care transitions. Appointment times can be aligned with childcare needs,
and our clinic is child-friendly.
Identify opportunities for process improvements and implement innovative solutions to
enhance the efficiency and effectiveness of the initiative
Other duties as assigned by the Practice Manager and Director of Operations
Share in after hour responsibilities if / as indicated

Qualifications:
Preferred current NYS Phlebotomy Certification
Previous experience managing grant programs and or research projects with the
demonstrated ability to manage meetings, create action plans and utilize cloud based
systems (preferrably electronic medical records), excel and word
Reports To: Director of Operations
