    • Responsible to the Agency Director and the Clinical Operations Manager at Carteret Health Care. Consults with Home Health and Hospice staff, Care Transitions and Case Management staff, referral sources, and community resources as appropriate.
  • DEFINITION OF POSITION

In this vital role, you are the "air traffic controller" for our Home Health and Hospice team-ensuring no patient or clinician ever feels unsupported. This is primarily an office-based position (limited field visits) perfect for an RN who excels at organization and clinical problem-solving.

      • Bridge the Gap: Act as the primary link between referral sources, families, and our field staff.
      • Expert Triage: Be the calm, clinical voice on the phone providing guidance when families need it most.
      • Operational Excellence: Manage the intake process and coordinate schedules to keep our care seamless.
      • Team Support: Act as the reliable "anchor" for our field clinicians, providing the office support they need to succeed.

    • The RN Patient Care Coordinator Home Health & Hospice is responsible for ensuring continuity between referral sources, patients and families, clinicians, and providers for the Home Health & Hospice programs in order to maximize quality care provided with exceptional compassion and respect, patient satisfaction, patient safety, comfort, and well-being.
    • The RN Patient Care Coordinator is responsible for coordinating referrals to ensure timely admission and appropriate patient care based on patient needs and / or physician orders.
    • This RN Patient Care Coordinator will act as an ongoing care coordination resource for current patients/families, potential patients/families, and for visit staff (nurses, aides, therapists, MSW, Chaplain, Dietitian, etc).
    • The RN Patient Care Coordinator communicates with other appropriate team members, Clinical Operations Manager, and Director.
    • The RN Patient Care Coordinator will work within the scope of the Nurse Practice Act regarding judgment, nursing skills, education.
  • QUALIFICATIONS
    • Professional
      • Current RN licensure in the State of North Carolina.
      • More than 2 years progressive clinical experience with a minimum of two years' experience in home and community based care preferred.
      • Evidence of participation in career development for professional growth through use of literature, research, and educational activities.
      • Excellent telephone etiquette and interpersonal skills.
      • Strong competency in computer skills, such as word processing, spreadsheets and e-mail (Microsoft Word, Excel, Access & Outlook).
      • Basic typing skills (minimum 35 words per minute). Appropriate and professional grammar and correct spelling is expected and essential.
