At Troy Rehabilitation and Healthcare Center, we're committed to providing exceptional care while creating a supportive, team-oriented environment for our employees. We are currently seeking a Full-Time Registered Nurse (RN) to join our dedicated nursing team.

This is an exciting opportunity for an RN who enjoys both direct resident care and playing an important role in welcoming new residents to our community. In addition to providing high-quality nursing care, this position will assist with new resident admissions, helping ensure a smooth, compassionate transition into our facility.

What You'll Do
  • Provide skilled nursing care in accordance with physician orders and individualized care plans.

  • Assist with the clinical admission process for new residents, including assessments, documentation, medication reconciliation, and coordinating a seamless transition into the facility.

  • Collaborate with physicians, therapy, social services, and interdisciplinary team members to ensure exceptional resident outcomes.

  • Assess, monitor, and document resident conditions while promptly communicating changes.

  • Administer medications and treatments safely and accurately.

  • Supervise and support LPNs and STNAs while fostering a positive team environment.

  • Maintain compliance with state and federal regulations and facility policies.

  • Build meaningful relationships with residents and their families through compassionate, resident-centered care.
