Full-Time Occupational Therapist (OT) – Skilled Nursing Facility (Queens, NY)

We are seeking a dedicated and motivated Occupational Therapist (OT) to join our rehabilitation team at a Skilled Nursing Facility in Queens, NY. This is an excellent opportunity to work in a collaborative clinical environment focused on maximizing patient function, independence, and quality of life.

Key Responsibilities

• Evaluate residents and develop individualized plans of care

• Provide skilled occupational therapy services and interventions to support patient goals and functional outcomes

• Supervise and support Occupational Therapy Assistants (COTAs), including clinical guidance and collaboration

• Maintain compliance with all facility policies, regulatory standards, and patient confidentiality requirements

• Complete accurate, timely, and compliant clinical documentation

• Provide patient and caregiver education as appropriate

• Ensure safe and effective treatment practices while monitoring patient response and progress

Qualifications

• Valid New York Occupational Therapist (OT) license

• Graduate of an accredited Occupational Therapy program

• Skilled Nursing Facility (SNF) experience preferred

• Familiarity with Rehab Optima / Net Health preferred
