Job description

Adecco Healthcare & Life Sciences is hiring a Health Care Occupational Case Manager in Chicago, IL! This role is a HYBRID opportunity. Please read below and apply with an updated resume for consideration:

Type:                  One-year contract with opportunity for extension or full-time offer

Schedule:              Monday – Friday, 6:45AM – 3:30PM. In Office on Tues, Weds, Thurs

Pay:                       $40.00 – $44.00 per hour based on experience

Responsibilities of the Health Care Occupational Case Manager:

·       Assessing patients' medical, psychological, and social needs.

·       Developing individualized care plans in collaboration with healthcare providers and patients.

·       Coordinating healthcare services, including medical treatments, therapies, and consultations.

·       Monitoring patients' progress and adjusting care plans as needed.

·       Providing education and support to patients and their families.

·       Advocating for patients' needs and rights within the healthcare system.

·       Communicating with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.

·       Evaluating the effectiveness of care plans and making recommendations for improvement.

·       Documenting patient assessments, care plans, and interactions.

·       Keeping abreast of healthcare regulations, policies, and best practices related to case management.

Qualifications for the Health Care Occupational Case Manager:

·       Active Registered Nurse license in Illinois

·       2 plus year of RN experience

·       Experience in Case Management

·       Compassionate and empathetic approach to patient care is essential
