Occupational Therapist – Home Health Case Manager

Seeking an experienced Occupational Therapist Case Manager to support Home Health services in the North Metro territory. This is a full-time, weekday-only contract with no on-call or weekend requirements—solid structure, consistent workflow, and a traditional home health model that still works.

The OT Case Manager will independently manage a home health caseload, providing occupational therapy services, coordinating patient care, and ensuring accurate documentation and compliance with home health regulations. The role requires strong clinical judgment, organization, and patient-centered care in the home setting.

Manage and coordinate a home health OT caseload

Perform initial evaluations, reassessments, and discharge planning

Develop and implement individualized OT care plans

Provide therapeutic interventions focused on ADLs, safety, and functional independence

Collaborate with interdisciplinary home health team members

Educate patients and caregivers on treatment plans and home safety

Ensure timely, accurate clinical documentation

Comply with regulatory, quality, and productivity standards

Adult and geriatric home health patients

Varying diagnoses requiring functional, safety, and independence-focused OT care

Active Occupational Therapist license (WI or compact, as applicable)

Home Health OT experience strongly preferred

Ability to work independently in patient homes

Strong communication and time-management skills

Must be willing to float to surrounding territories as needed

Leadership and operational support available
