What You'll Be Doing
  • Reviewing clinician OASIS accuracy and provide feedback to clinicians
  • Supporting clinicians with complex patient situations
  • Monitoring quality measures, regulatory compliance, and patient outcomes
  • Assisting with case conferences, care planning, and clinical decision-making
  • Collaborating with physicians, intake, scheduling, and operational leaders
  • Helping drive initiatives that improve patient satisfaction, quality metrics, and hospitalization rates

What You'll Need
  • Active Oregon Registered Nurse (RN) license
  • Current BLS certification (AHA)
  • Previous Home Health experience required
  • Experience with Homecare Homebase (HCHB) required
  • Strong OASIS knowledge and experience required
  • Previous clinical leadership or supervisory experience required
  • Excellent communication, coaching, and organizational skills
  • Valid driver's license and insured vehicle
  • Strong clinical judgment and a passion for developing others
