What You'll Be Doing
Reviewing clinician OASIS accuracy and provide feedback to clinicians
Supporting clinicians with complex patient situations
Monitoring quality measures, regulatory compliance, and patient outcomes
Assisting with case conferences, care planning, and clinical decision-making
Collaborating with physicians, intake, scheduling, and operational leaders
Helping drive initiatives that improve patient satisfaction, quality metrics, and hospitalization rates

What You'll Need
Active Oregon Registered Nurse (RN) license
Current BLS certification (AHA)
Previous Home Health experience required
Experience with Homecare Homebase (HCHB) required
Strong OASIS knowledge and experience required
Previous clinical leadership or supervisory experience required
Excellent communication, coaching, and organizational skills
Valid driver's license and insured vehicle
Strong clinical judgment and a passion for developing others
