Position Summary for Registered Nurse Case Manager (RNCM):

  • This is a full-time position available in San Francisco County
  • Plan, organize and direct home health care services, from initial evaluation and assessment, pain and medication management, and care plan development.
  • Ensure high quality patient experience and positive health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on patient needs.
  • Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA), and collaborate with the patient's primary care physician to ensure continuity of care.
  • Provide coaching and education to patients and their caregivers based on clinical diagnosis, empowering them to self-manage their condition, prevent complications, and improve quality of life
  • Registered Nurse Case Manager (RNCM) positions also available in San Mateo and Santa Clara County

Minimum Qualifications for Registered Nurse Case Manager (RNCM):

  • Current CA Registered Nurse (RN) license
  • Current CPR card for Healthcare Providers
  • 1 year of Registered Nurse (RN) experience in Home Healthcare, preferred
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
  • Self-directed and able to work with minimal supervision.
