By joining our team, you will play a critical role in advancing Food as Medicine initiatives under New York’s Section 1115 Medicaid Waiver program—supporting members with unmet nutrition and social care needs.

Role Overview

Epicured is seeking a Social Care Coordinator (Remote) to support Medicaid members through referral coordination, screening, and care navigation services within the Social Care Network (SCN).

Reporting to the SVP of Customer Service, this role is responsible for managing high-volume inbound and outbound call center activity, coordinating referrals, supporting member assessments, and ensuring seamless engagement in Epicured’s nutrition services.

This position is ideal for someone who thrives in a fast-paced environment, enjoys helping people navigate healthcare services, and is detail-oriented with strong communication skills.

Key Responsibilities

  • Manage inbound and outbound member communications within a virtual Five9 contact center environment (phone and email) in accordance with established service levels and quality standards

  • Conduct referral intake and eligibility screening aligned with NYS 1115 Waiver and DOH guidelines

  • Schedule and support completion of non-clinical assessments and ensure timely follow-up

  • Provide navigation to medically tailored nutrition services and other HRSN supports

  • Coordinate referrals across internal teams and external community-based organizations (CBOs) and Social Care Network (SCN) partners

  • Maintain accurate, timely, and compliant documentation within the SCN platform and internal systems

  • Support closed-loop referral processes, including tracking, follow-up, and confirmation of service delivery

  • Respond to member inquiries related to services, participation, and billing coordination

  • Adhere to all applicable Medicaid, HIPAA, and NYS DOH privacy, compliance, and documentation standards

Qualifications

  • High school diploma or GED equivalent required; associate’s or bachelor’s degree preferred.

  • Experience working in a virtual contact center environment (Five9 preferred)

  • Familiarity with NYS Medicaid, Social Care Networks (SCNs), and HRSN frameworks preferred

  • Strong communication, documentation, and organizational skills

  • Ability to manage high-volume interactions in a compliance-driven environment

  • Bilingual (English/Spanish) preferred

Preferred Qualifications

  • Experience in healthcare, care coordination, or patient navigation.

  • Language skills preferred (Spanish, French, Hindi, Georgian, Cantonese, or Mandarin Chinese).

  • Familiarity with Medicaid programs, social determinants of health (SDOH), or Section 1115 Waiver initiatives.

  • Priority will be given to applicants with a Community Health Worker or Licensed Clinical Social Worker qualification.
