This positionensures the highest degree of quality care by planning, organizing, and leadingthe overall operation of our care services in accordance with resident needs,government regulations and our internal policies and procedures.
EssentialResponsibilities:
  • Oversee care coordination for allresidents, including but not limited to; coordinates service plan, activatesappropriate resources and communicates with the physician or pharmacy regardingmedication changes/issues, follows up on residents admitted to the hospital andcompletes necessary paperwork
  • Completes medical assessments of eachnew admission and ongoing changes in condition. Provides recommendations toExecutive Director on new admissions
  • Manages medication administration,medication records and medication documentation in the MARs. Maintains residentdocumentation, medical management documentation and care plan documentation
  • Leads and delegates nursing tasks todirect care staff in compliance with the Nurse Practice Act and regulatoryrequirements
  • Continually seeks out and directquality improvement activities that focus on wellness, clinical management,medication management, risk management, staff training, and safety/surveypreparedness
