Responsibilities:

As a Home Health RN, you will:

  • Be a clinical leader. Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care.

  • Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.

  • Collaborate and coordinate. Work seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care.

  • Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.

  • Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.

  • Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.

Job Requirements

*This is a PRN Position*

Qualifications:

  • Graduate of an accredited nursing program with an active RN license in the state served.

  • Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).

  • Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.

  • Current CPR/BLS certification.

  • Valid driver’s license and reliable transportation.
