Change Makers LLC is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity, serving clients primarily from our Hospice Center located in Sparks, Nevada. Regular business hours will be the standard work schedule, with occasional evening or weekend commitments anticipated for special events or critical program development.

• Conduct thorough assessments to understand and evaluate the social, emotional, and psychological needs of clients, ensuring they receive appropriate support and services.
• Provide one-on-one and group counseling sessions, utilizing evidence-based therapeutic approaches tailored to meet individual client needs, with a focus on those facing chronic illness and end-of-life issues.
• Develop and implement comprehensive service plans that align with clients' personal goals and objectives, adapting as necessary to changes in their circumstances or well-being.
• Act as a strong advocate for clients by helping them navigate community resources, their rights, and access to needed services while addressing broader social justice issues that may affect their quality of life.
• Contribute to program development efforts by evaluating existing services and participating in grant writing and reporting to secure funding for future initiatives.
• Engage in collaboration with external partners, including community organizations and governmental agencies, to enhance service delivery and increase social impact.
• Participate actively in team meetings, training, workshops, and community outreach efforts to maintain a strong operational network and ensure comprehensive client care.

Your work will primarily focus on clients receiving hospice care, as well as their families, aiming to provide emotional and psychological support as they navigate challenging and often life-altering circumstances. Through compassionate counseling and advocacy, you will play a significant role in enhancing the quality of life for these individuals, fostering resilience, and facilitating meaningful connections within their families and communities.

• A Master's degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential for this position.
• A robust understanding of trauma-informed care, social justice principles, and advocacy practices is vital.
• Exceptional communication, empathy, and organizational skills to effectively liaise with clients, families, and community partners.

• Previous experience in a nonprofit organization or a related setting is highly preferred.
• Familiarity with case management software and electronic health record systems to facilitate service delivery and documentation.
• Experience in developing and implementing educational or community programs related to social work is a plus.

This position offers a competitive salary ranging from $75,000 to $100,000 annually, commensurate with experience, plus a sign-on bonus.
