Position Overview

Bay Area Community Health (BACH) is seeking a Community Health Worker for a full-time, permanent role supporting clinic operations in Glenview and Gilroy. This position is ideal for candidates passionate about serving low-income families, connecting patients with community resources, and thriving in fast-paced, mission-driven environments.

Assignment Details
  • Modality: Community Health Worker

  • Openings: 1

  • Schedule:

    • Monday: 10:00 AM–7:00 PM

    • Tuesday: 8:00 AM–5:00 PM

    • Wednesday: 8:00 AM–5:00 PM

    • Thursday: 8:00 AM–5:00 PM

    • Friday: 8:00 AM–5:00 PM

  • Hours per Week: 40

  • Contract Length: Permanent (no end date)

  • Start Date: ASAP

  • Charting System: EPIC

  • Interview: Amergis conducts interviews (Teams, 30 minutes, camera on)

Required Experience & Certifications
  • Compassion and desire to work in community-based settings serving low-income families

  • CHW certification required

  • Strong customer service skills

  • Knowledge of local community resources

  • Candidates with prior BACH employment will not be considered

Onboarding Requirements

Must be completed within 48 hours of offer management:

  • TB

  • Drug screen

  • Dress code BACH form (eStaff)

  • Physical

  • Hep B

  • MMR

  • Varicella

  • Tdap

  • Fingerprints

Compensation

Hourly Pay: $25–$27/hr (Non-OT role; 8-hour shifts, 40 hours/week)

The Non-Clinical Case Manager coordinates referrals, service planning, and client documentation for designated caseload(s).

Minimum Requirements:

  • Professional License in Behavioral Sciences, Human Services, or Social Services preferred
  • Bachelor’s Degree in Behavioral Sciences, Human Services, or Social Services fields preferred
  • Experience in Case Management, Behavioral Sciences, Human Services or Social Services preferred
  • Complies with all relevant professional standards of practice 
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable 
  • Current Health certificate (per contract or state regulation) 
  • Must meet all federal, state and local requirements 
  • Must be at least 18 years of age
