Under the supervision of a licensed pharmacist, this role supports high-quality, compassionate patient care while adapting to the dynamic healthcare environment. Responsibilities include obtaining and reviewing post-discharge medical records, navigating multiple electronic medical record systems (EMRs), and analyzing clinical documents such as labs, history & physicals, provider notes, and discharge summaries.

The position involves conducting outreaches to members to update medication lists, coordinating with providers to confirm care relationships, and following up on clinical recommendations. Additional duties include identifying medication discrepancies, reviewing prescription claims data, and preparing detailed reports for pharmacist review.

Candidates will collaborate across care teams to support coordination efforts and must be able to work both independently and effectively in a fast-paced, evolving setting.

Primary Responsibilities:

          Obtain medical records pertaining to recent discharge.

          Navigate multiple EMRs

          Review medical documents such as labs, H&P, provider notes, and discharge summaries

          Outreach directly to members post-discharge to obtain an updated medication list

          May outreach to providers to confirm patient/ provider relationship

          Follow-up with member provider on recommendations

          Identify medication discrepancies

          Review Rx claims data

          Prepare organized and detailed reports for pharmacist review

          May have cross-team communication for coordination of care

          Works independently

Required Qualifications:

  • High School Diploma/GED
  • Current, clear, and unrestricted Pharmacy Technician licensure in New Mexico
  • Possess a National Certification (PTCB)
  • 2 years of experience in pharmacy roles
  • Intermediate level of proficiency in the Microsoft Office suite, including Outlook, Teams, and Excel

Preferred Qualifications:

  • Retail pharmacy experience
  • Experience with EMR's
