Description:
Department: MS/ICU/IMC RN.
Certifications required include ACLS, BLS, and PALS.
The unit has 20 beds.
Nurse-to-patient ratios are 1:5 during the day and 1:6 at night for MS, 1:4 for IMC, 1:2 for ICU, and 1:3-4 for pediatric patients.
Common diagnoses include cases from the elderly population, cardiac issues (e.g., CHF exacerbation, Afib RVR), pulmonary conditions (e.g., pneumonia, COPD exacerbation), and various neurological complaints (e.g., falls, TIA, stroke).
Additional cases encountered involve skin conditions (e.g., cellulitis), gastrointestinal issues (e.g., GI bleed), genitourinary problems (e.g., UTI), orthopedic injuries (e.g., hip fractures), and sepsis cases.
Stable pediatric patients may include those with respiratory illness, postoperative conditions, and dehydration requiring IV hydration.
RNs will be required to titrate drips such as heparin, insulin, amiodarone, cardizem, PCA fentanyl, propofol, and norepinephrine.
There are Tele/Monitor Technicians monitoring telemetry, but RNs must be able to read rhythm strips as needed.
Charge nurses take patient assignments while house supervisors do not take assignments as they round throughout the building.
There are nurse aides available, with a CNAs-to-RN ratio of 1:8-10 on days and nights.
Respiratory therapy equipment includes CPAP, BIPAP, and various types of ventilators, with RTs available 24/7.
Shifts are 12 hours, running from 0700 to 1930 and 1900 to 0730.
Weekend and holiday requirements include every other weekend and holiday.
There may be on-call requirements based on census levels.
RNs may be required to float to other departments as needed.
Scrubs are navy for RNs.
Unit-specific orientation will include computer-based training videos and one to two days of orientation on the shift.
