Case Manager -Bilingual English/Spanish

The W Assisted Living Communities is seeking a compassionate and dedicated Case Manager to join our growing team. We are looking for someone with a proven track record of coordinating resident services and a genuine commitment to resident satisfaction and quality of care. In this role, you will be part of a collaborative, supportive team of healthcare professionals who are passionate about making a difference every day.

What You'll Do

- Uphold and maintain adherence to residents' Bill of Rights

- Serve as a key point of contact between residents, families, and care providers

- Conduct assessments and coordinate admissions and re-admissions from the community, hospitals, and nursing homes

- Coordinate and supervise clinical care for residents across the continuum

- Maintain accurate, clear, and timely documentation in ALIS in accordance with facility standards

What We're Looking For

- Minimum 1 year of experience as a Case Manager required; Long Term Care experience is a strong plus

- Bilingual in Spanish and English preferred

- Experience with ALIS or a similar EMR system is a plus

- Strong communication skills with the ability to work effectively with families and outside care providers

- Highly organized with a genuine passion for working with the senior population

- Team player who thrives in a multidisciplinary environment
