Job description

Who We Are
New Life Treatment Center (NLTC) is a 245G-licensed nonprofit organization providing substance use disorder treatment across southwestern Minnesota. Our Woodstock facility offers medical detox and residential treatment in an environment built on compassion, dignity, and the conviction that lasting recovery is possible for everyone.
We are a close-knit, mission-driven team that shows up every day because the work matters. If you're early in your social work career and looking to grow alongside people who are genuinely invested in your development and a place to work where what you do changes lives, we'd love to meet you.
The Role
We're looking for a Licensed Social Worker (LSW) to join our inpatient and residential treatment team in Woodstock. Working as an integral member of the interdisciplinary clinical team, you'll support clients through the social, relational, and systemic dimensions of their recovery, meeting people where they're at and helping them build the connections and resources they need to move forward.
This role is a great fit for someone who is organized, compassionate, and ready to do meaningful work in a structured clinical environment. You'll practice within the scope of your LSW license under the documented supervision of a designated LISW or LICSW to provide you with built-in support and mentorship.
> > This role is based at New Life Treatment Center. Employees in this position are employed by ElevaCare, which provides staffing support for New Life.
What You'll Do
  • Conduct psychosocial assessments and social history intake documentation in support of the 245G clinical team and individualized treatment planning
  • Provide individual and group-based supportive and skills-based social work services within your licensed scope
  • Coordinate community resources, referrals, and supportive services tailored to each client's recovery goals
  • Facilitate family engagement, collateral communication, and coordination with external providers and community partners
  • Develop and support individualized discharge plans in collaboration with the clinical team, ensuring continuity of care and connection to post-treatment services
  • Provide co-occurring or mental health-related supportive services within the scope authorized for a mental health practitioner and under documented supervision by a designated Qualified Mental Health Professional (QMHP)
  • Maintain accurate and timely clinical documentation
  • Fulfill mandatory reporting obligations and uphold client confidentiality under 42 CFR Part 2 and HIPAA

What We're Looking For
Required:
  • Current, active LSW license in good standing with the Minnesota Board of Social Work (BOSW)
  • Bachelor of Social Work (BSW) degree from a CSWE-accredited program
  • Minimum of two years free from problematic substance use
  • Ability to pass a Minnesota DHS background study
  • Strong written and verbal communication skills with the ability to maintain professional boundaries
  • Commitment to trauma-informed, person-centered, and culturally responsive practice

Preferred:
  • Experience in a substance use disorder, mental health, or co-occurring treatment setting
  • Familiarity with 245G licensing standards, ASAM Criteria, and evidence-based SUD treatment practices
  • Experience with electronic health record (EHR) documentation
  • Personal or professional connection to the recovery community

Compensation & Benefits
NLTC offers a competitive salary commensurate with experience and qualifications, along with:
  • Comprehensive health insurance
  • Paid time off and paid holidays
  • Continuing education support and professional development
  • Structured clinical supervision consistent with BOSW requirements - included as a core part of the role, not an afterthought
  • A workplace culture grounded in accountability, mutual respect, and genuine mission alignment
