• Conduct ongoing assessments to identify and address the unique needs of each patient, ensuring the delivery of tailored treatment plans.
• Maintain consistent and open communication with supervisors, direct care staff, families, and other stakeholders to track individual patient progress.
• Develop and manage a tentative schedule for appointments, home visits, and observations, routinely updating supervisors to streamline case management.
• Coordinate home visits, ensuring compliance with in-home observation requirements and facilitating thorough evaluations.
• Monitor the implementation of program plans, ensuring effective delivery of therapeutic interventions to clients.
• Engage actively in Interdisciplinary Team Meetings to collaborate on comprehensive care strategies and share insights on patient management.
• Document monthly and quarterly progress notes that provide clear updates on patient development and treatment goals.
• Organize and conduct training for staff to ensure competency in implementing program plans and maintaining high standards in documentation.
• Provide follow-up interviews, home visits, and observations to ensure continual support tailored to clients' evolving needs.
• Maintain organized patient records to facilitate transparency and accessibility for involved parties.
• Act as a liaison between the public and the mental health community to aid in developing support initiatives and fostering collaborative relationships.

In this role, you will work with a diverse range of clientele, including adults and youth from various backgrounds facing challenges such as trauma, relationship issues, substance misuse, and broader mental health concerns. Your expertise will help these individuals navigate their struggles, providing them with the necessary tools and support to lead fulfilling lives.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• Candidates must hold active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.
• A minimum of two years of counseling experience is necessary, preferably within a clinical context.

• Specialization in areas such as family therapy, couples counseling, substance misuse, or trauma informed care is highly desired.
• A strong understanding of legal and ethical considerations in mental health practice is essential.
• Demonstrated interpersonal skills and the ability to foster rapport with a diverse clientele are essential to success in this role.
• Experience in collaborating with healthcare teams to enhance patient outcomes is preferred.
• Proficiency in electronic medical record (EMR) systems and related technologies is advantageous.

• The expected salary for this position ranges from $90,000 to $110,000 yearly, commensurate with experience and caseload availability.
