POSITION TITLE:  Day Center Worker (C.N.A. I) (C.N.A. II)

DEPARTMENT:  Day Center                      REPORTS TO:  Nurse Manager

CLASSIFICATION:  Non-Exempt              DATE:  October 2013

PRIMARY RELATIONSHIPS:  Nurse Manager, Center Manager , Clinical Manager, Health Care Coordinator, Recreational Coordinator, Transportation Coordinator, Transportation Driver, Receptionist,  participants, family members and public.

OBJECTIVE:  Under the supervision of the Nurse Manager, responsible for providing and assisting participants of Senior CommUnity Care with activities of daily living, restorative and supportive care.  Assists with organization and implementation of recreational program.  Monitors health status and provides a safe environment for participants.  Functions as a member of Interdisciplinary Team.

ESSENTIAL FUNCTIONS:

Assists participants and/or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.

Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Recreational Coordinator.

Monitors and assists participants with meals at feeding table.

Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas.

Accompanies participants to medical and other appointments.

Performs and/or assists with laundry, including clothes for participants.

Assists the Drivers with the loading and unloading of participants from vehicles.

Demonstrates dependability through consistent compliance with scheduled work hours.

Participates in the activities of the Interdisciplinary Team as needed and carries out the plan of care as directed.

Notes changes in participant's physical or emotional health and communicates this information to the appropriate staff person.

Maintains participants' medical record and fulfills Senior CommUnity Care charting and reporting requirements.

Assists the Transportation Department with participants' take home supplies.

Participates in and supports Quality Improvement Initiatives.

Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.

Acts within the scope of his or her authority to practice.

Maintains the confidentiality of all organization procedures, results, and information about participants, clients or families.

Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

Maintains safe working environment.  Follows Senior CommUnity Care Safety policies and procedures.

Participates in continuing education classes and any required staff and training meetings.

Performs other duties as required or requested.

Qualifications

Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

Must have a valid North Carolina driver's license, proof of insurance and have means of transportation.

Education:  Certified Nursing Assistant - C.N.A I  or C.N.A. II

Experience:  Successful completion of nurse's aide, home health aide or equivalent training course or a minimum of one year of related experience in caring for impaired adults.

Skills and Knowledge:

  • Skilled in establishing and maintaining effective working relationship's with participants, co-workers and the public.
  • Basic knowledge in working with frail and elderly to support Activities of Daily Living (ADL).
  • Skilled in identifying and recommending problem resolution.
  • Ability to work independently with minimum supervision.
  • Ability to communicate clearly and effectively.
  • Ability to work effectively with culturally, economically, and educationally, diverse populations.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
