Job Summary and Responsibilities

We are seeking a skilled and compassionate Chaplain to join CommonSpirit Health at Home. The Chaplain will serve as an integral part of the multidisciplinary team providing spiritual and emotional support for patients, caregivers and staff.

  • Participates in interdisciplinary care conferences to assist in assessing the spiritual and emotional needs of patients and caregivers including psychosocial issues, adjustment to illness or disability, status of religious/spiritual coping, supporting others in the process of identifying and applying their own values in decision making.
  • Demonstrates sensitivity to the needs, customs, or feelings of patients and caregivers.
  • Makes home visits to attend to patients at the end of life.
  • Provides support and comfort to the dying patient and caregivers, as well as prayer when requested by patient or caregiver.
  • Participates in bereavement and follow-up services.
  • Provides consultation services to develop individual plans of care addressing patients’ and caregivers' spiritual and emotional needs.
  • Educates field and office staff in how to assess and address spiritual and emotional needs of patients and caregivers.
  • Identifies resources to assist patients and families address spiritual and end-of-life needs.
  • Completes and submits all required documentation in an accurate and timely manner.
  • All other duties as assigned.

Job Requirements
  • Master’s Degree in Theological Studies or a related field.
  • Must have four completed units (1,600 hours) of appropriately accredited Clinical Pastoral Education.
  • Certification by a professional certifying agency or equivalent, or ecclesiastical endorsement required; Certification by NACC or APC or eligibility for certification, preferred. 
  • Minimum of One year Chaplain experience required.
  • Must exhibit a high level of interpersonal, facilitative and communication skills and ability to do theological reflection.
  • Valid Driver’s License is required.
