Job description
Healthcare Talent is assisting our client in their search for an accomplished Clinical Social Worker for their Orange facility. The ideal candidate will have experience working with the senior population and be well versed in their needs and how best to facilitate comprehensive care.
- Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
- Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs.
- Modify treatment plans to comply with changes in clients' status.
- Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
- Coordinate 24-hour care delivery and provide individual and family counseling for participants
- Develop and lead group counseling and activities
- Develop and lead caregiver counseling and activities
- Assist in the completion of advance directives, mortuary arrangements, funeral planning and/or facility placement
- Consult with, advise and encourage effective relationships with staff to ensure cohesive care
- Perform home visits to private homes of participants; frequent travel in Orange County
- Master's degree in Social Work from an accredited college or university
- Minimum two years experience in a health related area
- Minimum one year experience working with the frail or elderly population; experience must be within the last three years
- Valid California drivers license and vehicle or other approved means of transportation; an acceptable driving record is required.
- Bilingual Spanish required
