Seeking caring, compassionate and experienced RNs to join our team.

We currently have full-time Weekday and PRN positions available.


WEEKDAY FULL-TIME RN – This is our full-time, Monday – Friday RN position. To remain eligible for full-time status, all nurses are required to complete a minimum of 30 productivity points a week (on average). However, we expect all full-time staff to be available Monday-Friday 8:30-4:30 (8 hours a day) to ensure patient needs are met and addressed timely. Our full-time RNs typically perform 3-6 visits a day depending on the type of visits. This position does require participation in our on-call rotation. Depending on how many nurses we have in the rotation, will determine how often you are on-call. We have 2 compensation options for this position, Salary and PPV. Both include additional compensation for mileage reimbursement, on-call pay and on-call visits performed.


PRN/PPV OPTION - This position is an as needed, pay-per-visit position. This position does require participation in our on-call rotation. You would be paid for each visit performed, on-call, on-call visits and mileage reimbursement.

We are on the search for RN to join our team. This RN will coordinate, evaluate and plan an individualized care plan for patients who are receiving home health. The qualified candidate will be a team-player and have an in-depth understanding of home health care services and have experience coordinating numerous concurrent programs at once.

Responsibilities:

  • Prepare and implement a treatment plan for patients in cooperation with physicians for patients.
  • Provide direct patient care in home settings, including but not limited to wound care, catheterization, tube feeding, and IV infusion as needed.
  • Conduct thorough patient assessments and care to patients.
  • Maintain accurate and up-to-date electronic health records (EHR) while adhering to HIPAA regulations.
  • Collaborate with healthcare teams to develop and implement individualized care plans.
  • Educate patients and families on health management and support their emotional well-being.
  • Travel to various locations to deliver care, ensuring flexibility and adaptability in diverse environments.
  • Participate in rotating on-call schedule.

Qualifications

  • Current RN License
  • Experience in home health and/or hospice care preferred
  • Proficient in managed care, patient assessment, and discharge/death planning
  • Strong knowledge of medical terminology, HIPAA regulations, and physiology
  • Experience in critical care, geriatrics, and hospital settings preferred
  • Ability to develop and implement effective care plans for patients
  • Familiarity with inpatient, outpatient, and post-acute care environments
