Summary
Florida Health Care Plans is seeking an APRN with an Autonomous Practitioner License to join our Palm Coast facility located at 309 Palm Coast Pkwy NE. Office hours are Monday - Friday 8-5, outpatient only, no weekends, call is light and by telephone only. The APP will provide necessary and appropriate primary and urgent care services, within the scope of their training and experience. Responsibilities and expectations to include the following:
• Deliver patient-centered, compassionate care
• Provide comprehensive medical assessments and treatments
• Develop personalized patient care plans
• Continuously enhance clinical knowledge
• Educate patients on disease prevention and health maintenance
• Collaborate with nurses, medical assistants, and other healthcare providers to ensure continuity of care.
• Complete documentation, billing, and coding as required by healthcare regulations and standards.
• Participate in quality improvement initiatives within the practice.
• Attend staff meetings, training, and continuing education as required
• Participate in clinically driven initiatives to include annual complete physicals, quality and risk adjustment metrics
Qualifications
• APRN Autonomous Practitioner License
• DEA License
• Minimum 3 years' post licensing experience, working independently, in Family Medicine.
• Graduate of an accredited APRN / PA program
