Job description

Rate of Pay: $35.54/hour + DOE

Summary
The Community Health Project Coordinator supports the launch and ongoing operations of the Tahoe Forest Health System's Mobile Health Clinic. Working in partnership with the Mobile Health management team, this position coordinates project planning, community partnerships, communications, and program evaluation to ensure successful implementation and long-term sustainability. The position also supports other Community Health Department programs and initiatives as assigned.

Essential Duties and Responsibilities
  • Serves as the primary liaison between TFHS departments, Mission Mobile, and community stakeholders to support implementation and ongoing operations.
  • Coordinates implementation activities, meetings, project timelines, and deliverables for the Mobile Health Clinic.
  • Develops and maintains project documentation, meeting agendas, and action items.
  • Coordinates community partnerships, host site, schedules, outreach, communication, and public facing program information.
  • Provides project information to stakeholders so that project decisions can be made.
  • Assists with the development of host sites Memoranda of Understanding.
  • Monitors program metrics, data collection processes, and reports to support continuous program improvement.
  • Supports culturally responsive outreach to improve access to care.
  • Provides leadership to the project team and stakeholders in the following areas:
    • Builds solid working relationships
    • Encourages decision-making
    • Resolves conflicts in a timely manner
    • Supports Community Health Action Plan (CHAP) workgroups and other Community Health initiatives as assigned.
  • Demonstrates System Values in performance and behavior.
  • Complies with System policies and procedures.
  • Performs other duties as assigned.

Minimum Education/Experience
