Job Description
Part-time:16 hours per week and occasional other hours providing on-site direct care/supervision plus one weekend per month on-call.
Qualifications
Qualifications:
• Current MN RN license
• 3-5 years experience working with residents with memory impairment
• Experience working with MDH Comprehensive Home Care license in a residential care setting and/or home care preferred
• Excellent assessment skills
• Ability to transfer knowledge to Home Health Aides/CNAs
• Strong team building and mentoring skills
• Strong organizational and analytical skills
• Ability to make independent decisions
• Experience with aging in place philosophy of care
• Experience with end-of-life process preferred
• Demonstrates ability to communicate effectively verbally and in writing
