Today, we are a 12-bed acute care federally designated Critical Access Hospital and a 50-bed long-term care facility, which also includes a home healthcare agency, an 8-bed assisted living facility, and ambulance services. Our campus includes a medical clinic in Hallock and a satellite clinic in Karlstad, MN. Job Summary:
A Registered Nurse (RN) in long term care is responsible for providing comprehensive nursing care to patients in a long-term care setting. This role involves assessing patient needs, developing care plans, administering medications, and collaborating with healthcare teams to ensure optimal patient outcomes. The registered nurse will also educate patients and their families about health management and support them in daily living activities.
Primary Responsibilities Include:
  • Supervise and provide directions of Licensed Practical Nurse (LPN), Certified Med Aide (CMA), and Certified Nursing Assistants (CNA)
  • Monitor and assess resident health needs and follow care plans
  • Administer medications and manage treatments
  • Provide wound care and follow infection control practices
  • Collaborate with healthcare teams to ensure optimal outcomes
  • Document nursing care, observations, and resident responses accurately and timely in EHR
  • Maintain compliance with all state and federal regulations, including those related to infection control, resident rights, and safety standards
  • Promote positivity, compassionate environment that support resident dignity and independence
Job Requirements:

  • Proficiently be able to read, write, speak, and understand the English language

Qualifications:
  • Graduate of an accredited nursing program leading to licensure as a Registered Nurse (RN)

  • Complete TB testing within the first 3 weeks of employment

Job Schedule/Location(s):
  • Hallock, MN
  • Shift options: 6:00am – 2:30pm, 2:00pm – 10:30pm OR 10:00 pm – 6:00am
  • Every other weekend and holiday rotation required
