Are you an experienced Registered Nurse with a background in home health who enjoys working independently and making a meaningful impact in the community? This long-term opportunity as a Home Health Nurse Assessor allows you to conduct in-home assessments that help ensure patients receive the care and services they need.

This is a Hybrid set-up, only LOCAL candidates may apply.

Rate increase valid for caregivers submitted during May and starts in June


The schedule is flexible that you can start earlier in the day and also get 3 assessments per day versus 2 visits. You can also work on weekends.

Minimum Requirements

Candidates must have at least 2 years of home-based healthcare experience within the last 10 years, including one or more of the following:

  • Field work with a Certified Home Health Agency (CHHA)

  • Work with a Licensed Home Care Services Agency (LHCSA)

  • Experience with Managed Long-Term Care (MLTC) programs

  • Conducting home-based patient assessments (e.g., OASIS)

  • Providing primary care to homebound patients

  • Care management for individuals receiving home and community-based services, hospice, or assisted living

  • Home care experience as an LPN or Home Health Aide caring for at least 2 patients per week
