Job description

BASIC FUNCTION:   
The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals who have traumatic brain injury. 

SPECIFIC DUTIES/RESPONSIBILITIES:
•    Implements all aspects of Minds Matter philosophical principles for effective service delivery.
•    Adhere to professional, ethical, and legal standards of practice.
•    Provide direct supervision of Physical Therapy Assistants (PTA) in accordance with state of Kansas provider manual.
•    Assist individuals to identify their needs and develop appropriate physical compensation techniques to accommodate the consequences of their brain injury.
•    Assess the need and recommend adaptive or assistive technology as needed by consumer.
•    Provide specialized physical therapy and intervention programs.
•    Develop staff and family training on specific physical therapy strategies to increase physical activity. 
•    Coordinate the dissemination of treatment plans to the team.
•    Integrate employment, educational and independent living goals into therapy.
•    Work closely with the consumer and Physical Therapy Assistant (PTA) to ascertain goals. 
•    Provide services only as designated on each individual consumer’s plan of care.
•    Work directly with consumers to improve skills in all aspects of their life goals.
•    Maintain and turn in all documentation at assigned times.
•    Educate families about the consequences of brain injury and communicate with them about goals and progress as needed.
•    Attend all trainings and workshops to maintain ability to provide the most up to date, effective and evidence-based practice.
•    Provide effective documentation that is legible, timely and appropriate to services provided to consumers.
•    Be aware of departmental goals and objectives and take an active role in helping the department to attain goals.
•    Make initial contact within 48 hours of receipt of referral to schedule evaluation, which must be completed within 7 days of receipt of referral.
•    Set goals for consumers with input from Physical Therapy Assistants/consumers that Physical Therapy Assistants/Physical Therapist team will help them achieve. PTAs are not able to set goals. 
•    Provide regional rehab directors with quarterly summaries related to goal attainment.
•    Practice Minds Matters policy and procedure requirements including completion of all required documentation in a timely manner. 
•    Establish and maintain a professional rapport with all consumers, co-workers, referral agents and representatives of other community agencies and services, including attendance at planning meetings, etc.
•    Promote public relations throughout the agency and community for Minds Matter, LLC.
•    Demonstrate a willingness to serve on agency and interagency task groups as requested.
•    Perform other duties as assigned
•    Provide care and maintenance for Minds Matter issued devices. 

SUPERVISORY RESPONSIBILITY:
Physical Therapy Assistant
 

MINIMUM EDUCATION AND EXPERIENCE:
Must be a graduate of an American Physical Therapy Association (APTA) accredited school of physical therapy
Licensed as a Registered Physical Therapist in the state of Kansas

REQUIRED SKILLS AND ABILITIES:
High degree of sensitivity toward all people
Ability to effectively plan, coordinate and assist with all aspects of therapy service delivery
Knowledge of physical therapy principles, theory, and methods
Ability to select and administrator appropriate assessment tools and interpret results of assessments
Excellent time management skills, and problem-solving skills
Excellent verbal and written communication skills
Creativity
Ability to use electronic equipment i.e. IPads and applications
