Company Overview
Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community. Services include 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults, as well as outpatient services such as medication management, case management, and counseling.
Job Summary
The FACT Assistant Team Leader assists the FACT Team Leader administer and direct the Florida Assertive Community Treatment (FACT) Program, a clinical team, which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to persons with serious mental illness. Promotes recovery, empowerment, self-determination and decision-making for each FACT participant.
The FACT (Forensic Assertive Community Treatment) Program is a multidisciplinary clinical team that provides comprehensive, community-based services to adults with severe and persistent mental illness. The program is designed to reduce hospitalization, increase community tenure, and enhance independence, autonomy, and quality of life.
FACT team members provide a wide range of services, including advocacy, crisis intervention, assessments, treatment planning, case management, medication support, and coordination of care. Additional services include community referrals, symptom education, vocational and educational support, social skills development, and assistance for families and caregivers.
Most services are provided in the community, with approximately 15% of time spent in the office. Company vehicles are available; however, team members must also use their personal vehicles as needed and are eligible for mileage reimbursement.
The FACT team operates during and outside of standard business hours, including rotating weekend, holiday, and on-call responsibilities.
Responsibilities
  • Provides treatment, rehabilitation, and support services in accordance with the FACT model, which is based on the Program for Assertive Community Treatment (PACT) manual
  • Assures the accuracy, completeness, and confidentiality of clinical records
  • Conducts mental health status assessments
  • Coordinates referrals to community providers and support services
  • Provides assistance with activities of daily living, transportation, communication, social and interpersonal relationship skills
  • Provides education related to symptoms, medications, and mental health issues

Assists the FACT Team Leader in:
  • Monitoring and directing the services provided by the Florida Assertive Community Treatment (FACT) program.
  • Directing the day-to-day clinical operations of the FACT Team
  • Scheduling staff work hours to ensure adequate coverage for the day, evening, weekend, and holiday shifts and on-call hours.
  • Coordinating the daily organizational meetings to ensure completion of the work for the day and ensure identified needs are met.
  • Coordinating the admission process and the treatment, rehabilitation, and support services of the program.
  • Reviewing documentation to ensure timely and appropriate treatment to persons served, which meets State requirements and MHRC Medical Records Standards.
  • Providing training and orientation to new staff, and ensuring compliance with staff development plans.

  • Accessible on desktop or mobile for flexibility across different work settings

Qualifications
  • Master's degree in Human Services or a related field required.
  • Previous experience working with adults with severe and persistent mental illness required.
  • Experience with DBT therapy techniques preferred.
  • Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
  • Proficiency in Microsoft Office, Outlook and the use of the Internet.
  • Must meet MHRC/RBHS driver requirements, including a valid Florida driver's license, required insurance coverage, and acceptable driving record.
  • Ability to work in varied settings including community, hospital, and residential environments.
  • Strong clinical judgment with ability to respond to emergencies and high-risk situations.
  • Strong leadership and communication skills with ability to collaborate across teams and systems.

Mental Health Resource Center, Inc. (MHRC) is a nonprofit organization that provides a wide range of behavioral health and social services to the community. Included in MHRC's service array are 24-hour emergency services and inpatient psychiatric services for children, adolescents, and adults. Outpatient services include medication management, care coordination, case management, intensive case management team services, counseling, psychosocial rehabilitation, an adult therapeutic family program, state hospital liaison services, mental health court, multidisciplinary forensic teams (MFT), jail-based diversion services, a Co-Responder Program with the Jacksonville Sheriff's Office, the Link-to-Life suicide prevention program, and services that assist individuals in obtaining benefits such as Social Security Income, Social Security Disability Income, Medicaid, food stamps, and housing. In addition, UF Health Jacksonville contracts with MHRC to manage the hospital's adult inpatient psychiatric unit.
MHRC also operates eight Florida Assertive Community Treatment (FACT) programs for adults with severe and persistent mental illness, located in Clearwater, Gainesville, Jacksonville, Kissimmee, Rockledge, Tampa, and Winter Haven, as well as a FACT Lite program providing Linking, Advocating, Treating, Transitioning, Empowering & Recovery Support (LATTERS) services in Jacksonville. The two FACT teams located in Jacksonville serve residents of Duval, Clay, and Nassau counties.
