Step Into a Role That Truly Matters: Primary Care Nurse Practitioner in Connersville, IN

Are you a passionate, forward-thinking Nurse Practitioner ready to make a meaningful difference? At Meridian, you'll use your expertise to deliver life-changing care while collaborating with an exceptional multi-disciplinary team. You will actively collaborate with treating physician in assessing and monitoring patients' physical and mental health status. Monday-Friday, 8a-5p or the possibility of four 10 hour days.

What You'll Do As a Primary Care Nurse Practitioner

Your day-to-day work will empower patients and strengthen community health. You will:

  • Deliver comprehensive nursing care using advanced clinical knowledge and evidence-based practice
  • Conduct physical exams across diverse populations using a structured, developmental approach
  • Make differential diagnoses and identify both medical and non-medical contributors to health
  • Create individualized treatment plans that improve outcomes and support long-term wellness
  • Collaborate with patients and families to build meaningful, personalized health plans
  • Ensure accurate, timely documentation aligned with Meridian standards
  • Champion ethical, compliant care in accordance with all regulations
What You Bring

We're excited to meet candidates who have:

  • A Master's degree from an accredited Nurse Practitioner program
  • An active (or eligible) Indiana RN license in good standing
  • Nurse Practitioner certification (Family Practice, Psych/Behavioral Health, or Pediatrics)
  • Eligibility or active prescriptive authority/DEA licensure
  • Preferably 2+ years of related experience
  • Community health or mental health experience (a plus!)
  • A valid driver's license, reliable vehicle, and current auto insurance
