Community Care Coordinator

We are seeking a compassionate and detail-oriented Community Care Coordinator to oversee daily resident care, assist with Activities of Daily Living (ADLs), and manage caregiver scheduling. This role is perfect for a Certified Nursing Assistant (CNA) with strong leadership and organizational skills.

Key Responsibilities:

  • Assist residents with ADLs including bathing, dressing, grooming, toileting, and mobility.
  • Plan and post monthly schedules for CNA, Caregiver, Med-Tech, and Nursing staff.
  • Adjust staffing for call-offs and emergencies to ensure coverage.
  • Support recruitment efforts by scheduling interviews and assisting with onboarding.
  • Greet visitors, answer calls, and coordinate communication between residents, families, and staff.
  • Monitor resident health, respond to emergencies, and document changes.
  • Maintain accurate health records, medication logs, and supply inventories.
  • Participate in resident care conferences, activities, and welcome/orientation programs.
  • Ensure cleanliness and organization of resident rooms, dining areas, and common spaces.

Qualifications:

  • High school diploma or equivalent.
  • CNA Certification (required).
  • CPR/First Aid Certification.
  • Strong communication, computer, and organizational skills.
  • Ability to work in a fast-paced, team-oriented environment.
  • 12 hours of annual continuing education (provided in-house).
