Job description

This role is responsible for coverage of open shifts in all ambulatory pharmacy programs. The ambulatory team pharmacist will operate at a variety of facilities working in both a retail and clinical role. This position is responsible for working with a multi-disciplinary medical care team to evaluate therapeutic assessments and selected disease management focusing on patient outcomes, while maintaining cost. The major duties of the ambulatory team pharmacist will include staffing at the Holzer Family Pharmacies and will be expected to enter, fill and dispense medications according to all state board of pharmacy requirements in addition to duties of continuation of ambulatory clinical pharmacy services such as; Meigs anticoagulation clinic, Transitions of Care services, conducting chart reviews, attending department specific meetings, leading classes for the employee diabetic program, providing pharmaceutical information to staff pharmacist, technicians, patients and allied health partners and the education and development of students and residents.
Education, Work Experience and Licensure:
• Graduate of a College or School of Pharmacy, accredited by the American Council on Pharmaceutical Education with a minimum of a Bachelor's Degree in Pharmacy.
• Doctor of Pharmacy Degree preferred.
• Maintains licensure in good standing as a Registered Pharmacist in the State of Ohio.
• Board certification required for advance practice clinical activities or settings.
• Must be eligible for licensure as a pharmacist in Ohio.
• Hospital experience preferred.
