Homecare Navigator 

About the Role 

Are you passionate about helping people remain safe, supported, and independent at home? Focus Healthcare is looking for a Homecare Navigator to serve as the primary relationship manager and advocate for our clients — from their very first inquiry through every stage of their care journey. 

You'll own the intake and assessment process, lead new clients through a smooth and welcoming onboarding experience, and stay their trusted point of contact for the life of their service. You'll be the face of Focus Healthcare in the community — building relationships with clients, families, caregivers, and state service coordinators, and making sure every care plan is carried out the way it should be. 

What You'll Do 

Client Intake & Assessment 

  • Serve as the first point of contact for new client inquiries and conduct in-home, non-clinical assessments. 

  • Determine when a case needs escalation to a nurse, and route it accordingly. 

  • Build accurate, thorough care plans and guide clients through intake documentation and next steps. 

Onboarding & Positive Starts 

  • Guide new clients and families through onboarding, setting realistic expectations for scheduling and service. 

  • Partner with scheduling and operations to support a strong caregiver match and timely start of care. 

  • Stay in close contact during a client's first weeks to catch and resolve issues early. 

Retention & Relationship Management 

  • Build lasting, trusted relationships with clients and families through regular visits and check-ins. 

  • Spot early warning signs of dissatisfaction or risk, and create proactive retention plans. 

  • Own client concerns through to resolution — and follow up to confirm they're truly resolved. 

    Quality Assurance & Care Coordination 

    • Confirm caregivers are following the established plan of care through home visits and service reviews. 

    • Keep care plans current and document visits, calls, and concerns promptly and accurately. 

    • Partner closely with operations, scheduling, quality, and caregiver teams to resolve service gaps. 

    Community & Referral Partnerships 

    • Build strong working relationships with state service coordinators and referral partners. 

    • Represent Focus Healthcare professionally, reinforcing our reputation for responsive, person-centered care.

    • Timely, thorough client intakes and assessments 

    • Strong client retention and satisfaction 

    • Consistent completion of home visits and quality calls 

    • Fast, effective resolution of client concerns 

    • Reliable documentation and care-plan compliance 

      What We're Looking For 

      • Experience in home care, social work, healthcare, human services, case management, client relations, or account management preferred. 

      • Background in intakes, assessments, or care plan development strongly preferred. 

      • Experience with Medicaid waiver programs, service coordinators, older adults, or family caregivers a plus. 

      • Excellent relationship-building, communication, and conflict-resolution skills. 

      • Comfortable working independently in the field while staying closely connected to the team. 

      • Strong organizational skills — able to juggle multiple clients, visits, and deadlines. 

      • Valid driver's license, reliable transportation, and willingness to travel throughout the service area. 

      • Able to pass required background checks and employment screenings.

      Compensation & Benefits 

      • Salary of $45,000–$55,000 annually 

      • Annual performance bonus of up to $10,000 (paid quarterly) based primarily on client retention

      • Monthly Cellphone Reimbursement

      Work Environment 

      This hybrid, field-based role spends about 70% of time in the community — conducting assessments, visiting clients, meeting families, and partnering with service coordinators — with the remaining time spent remotely or in-office on documentation, quality calls, and reporting.
