Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. 
 
We are now hiring a Full-Time Medical Coordinator in Queens, NY. The Medical Coordinator will plan, track, monitor, and ensure that all clients’ needs are being met within appropriate time frames. The Medical Coordinator will be responsible for scheduling all annual, follow-up, and emergency medical appointments. This additionally includes communicating with medical professional staff, program specialists, supervisors, and program directors. This position requires driving to medical appointments - valid driver's license is mandatory.
Starting pay is $20.00/ per hour

Responsibilities
  • Schedule all client medical appointments to state mandated licensing regulations
  • Observe and monitor the client’s status through paperwork and follow-up appointments
  • Observe and track clients' physical care including, but not limited to, annual and emergency appointments
  • Provide accurate, timely, and consistent documentation of client medical appointments in the appropriate records
  • Attend all pertinent medical appointments that need to be addressed more clearly and precisely to achieve the best results for the individual
  • Communicate with supervisors, co-workers, and medical professionals’ regarding clients’ status
  • Additional tasks and responsibilities may be assigned
  • Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation
  • Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
Minimum Requirements
  • High school diploma or GED; bachelor’s degree preferred
  • Valid driver’s license with a satisfactory driving record
  • Complete all state and agency required training per state guidelines
  • Ability to establish a comfortable and supportive relationship with individuals receiving supports
  • Ability to exercise good judgment and remain calm in crisis situations
  • Excellent verbal and written communication skills
