Job description

Position Title: Medical Records

Department: Operations

Classification: Hourly

Reports To: Regional Administrator

Position Overview:

The Medical Records Clerk is responsible for scanning, pulling, and auditing patient charts, referral tracking,Β managing the incoming fax server; verifying insurance coverage and printing encounters for next day scheduledΒ appointments. Participates in the creation and management of patient records and files using ECW as theΒ Electronic Health Record System. The Medical Records Clerk is responsible for projecting a positive firstΒ impression of the organization by communicating effectively with co-workers and the public, and greetingΒ patients and other visitors, determining their needs, and directing them accordingly to the appropriate party withΒ complete confidentiality.

What You Will Do:

  • Perform duties per AAAHC, Meaningful Use guidelines, and certified EHR systems.
  • Cross-train, work front office at designated sites, and travel to outlying clinics as needed.
  • Call patients 2 days in advance to remind them of upcoming appointments.
  • Collect co-pays, copy insurance cards, and remind patients of outstanding balances.

  • Ensure patient record accuracy and availability by pulling and alphabetizing charts prior to visits.
  • Assist medical personnel in locating missing medical records.
  • Provide phone and clerical support, including messaging and typing for providers.
  • Manage scheduling, enter/update patient info, and schedule/confirm appointments.
  • Coordinate billing and records by printing encounters, collecting payments, and running reports.
  • Maintain record accuracy by filing lab, ER, and other reports in patient charts.
  • Process formal medical record requests from third parties with proper authorization and logs.
  • Report to work on time and prepared for duties.
  • Follow up on missing medical records from ER, labs, and providers, and classify/label charts.
  • Create medical charts for new patients.
  • File transcribed reports into medical charts as a member of the Operations Team.
  • Support clinic operations by assisting the telephone triage nurse, faxing, and handling Clinic Manager requests.
  • Pursue continuing education to update professional skills.
  • Deliver respectful, confidential, and high-quality customer service.
  • Perform other duties per job description and ethics policy.

Minimum Qualifications:

  • Education: High School Diploma or GED equivalent.
  • Experience: Minimum of two (2) years' experience, with at least one (1) year in a related field.

Preferred Qualifications:

  • Education: AA Degree or equivalent academic study.
  • Experience: Minimum of one year experience in medical records working in a hospital or clinic setting.
  • Bilingual, Spanish/English.
  • Experience with medical terminology and health insurance claims, Medicaid, and Medicare.

Physical Requirements:

  • Balancing, bending, lifting, carrying, climbing and pulling up to 20 lbs.
