Job description

Primary Care Physician Essential Responsibilities

Fully engaged in the concept of "Integrated team-based care" model

Focusing on outpatient medicine, continuity of care, health maintenance, and disease prevention

Demonstrates both autonomy and proficiency in performance of comprehensive health assessments and communicate Plan of Care with Patients and/or their families

Proficient in diagnosing, treating and managing the Medicare Advantage population with multiple comorbidities

Effectively communicates with all members of the care team and facilitates a medically appropriate and effective plan of care to avoid unnecessary duplication of testing, and fragmentation of care

Observes the scope of practice in the state in which he or she is licensed and working in

Schedule Requirement

This is a FULL-TIME or PART - TIME position

Monday to Friday 8:00 am - 5:00 pm

Eligibility & Qualifications

Ability to use Electronic Medical Record (EMR) system to document patient encounters

Knowledge of ICD-10 Coding and STAR measure attainments

Effective communication and interpersonal skills

Demonstrated ability to problem-solve complex and multifaceted situations

Strong organizational, and task prioritization skills

Education/Experience

Medical Doctorate (M.D.) or Doctor of Osteopathic Medicine (D.O.);

Family Medicine, Internal Medicine, or Geriatric Medicine preferred

Medical License in the state of Texas

DEA License

Up-to-date CPR Certification (BLS, ACLS, and PALS)
