Job description

Primary Care Physician Essential Responsibilities:ย 
  • Fully engaged in the concept of โ€œIntegrated team-based careโ€ model
  • Focusing on outpatient medicine, continuity of care, health. maintenance, and disease prevention.
  • Demonstrates both autonomy and proficiency in performance of comprehensive health assessments and communicate Plan of Care with Patients and/or their families.
  • Proficient in diagnosing, treating and managing the Medicare Advantage population with multiple comorbidities.
  • Effectively communicates with all members of the care team and facilitates a medically appropriate and effective plan of care to avoid unnecessary duplication of testing, and fragmentation of care.
  • Observes the scope of practice in the state in which he or she is licensed and working in.

Schedule Requirement:ย 
  • This is a FULL-TIME or PART - TIME position.
  • Monday to Friday 8:00 am - 5:00 pm.

Eligibility & Qualifications:ย 
  • Ability to use Electronic Medical Record (EMR) system to document patient encounters.
  • Knowledge of ICD-10 Coding and STAR measure attainments
  • Effective communication and interpersonal skills.
  • Demonstrated ability to problem-solve complex and multifaceted situations.
  • Strong organizational, and task prioritization skills.

Education/Experience:ย 
  • Medical Doctorate (M.D.) or Doctor of Osteopathic Medicine (D.O.);ย 
  • Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
  • Medical License in the state of Texas.
  • DEA License.
  • Up-to-date CPR Certification (BLS, ACLS, and PALS).
