This is a primarily telephonic role — the majority of your patient interaction happens by phone, with limited face-to-face contact. If you are a strong communicator who documents thoroughly and genuinely cares about patient follow-through, this position is a good fit.
What You'll Do
- Manage transitional care for patients following hospital or facility discharge, including timely outreach and follow-up
- Support medication adherence by reviewing regimens with patients, identifying barriers, and coordinating with the care team
- Deliver chronic care management (CCM) services through regular check-ins on patients with ongoing conditions
- Document every patient interaction thoroughly and accurately in our care management system
- Coordinate with providers and practice staff to address patient needs and close care gaps
Required Qualifications
- Active certification or licensure as a Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) in the state of Georgia
- Strong phone communication skills and a patient-centered, empathetic approach
- Proficient typing and computer skills, with the ability to document clearly and completely in real time
- Comfortable working independently in a structured, phone-based environment
- Reliable, detail-oriented, and able to protect patient privacy in accordance with HIPAA
Preferred Qualifications
- At least 2 years of experience in a clinical, care management, or patient-facing healthcare role (candidates with less experience will be considered)
- Prior experience with care management, care coordination, or chronic disease management
- Familiarity with electronic health records (EHR) or care management platforms
Company Description
Connect Care Management provides outreach to patients enrolled in our programs. These programs are chronic care management, remote patient monitoring, behavioral health integration, annual wellness visits, and hospital follow ups. A majority of our work is done over the phone and little face-to-face interaction is had with our patients.
