Position Summary

The Registered Nurse (RN) provides direct patient care and supports the physician in delivering the highest quality concierge-level healthcare. This role ensures a positive membership experience by coordinating clinical care, assisting with patient education, and supporting front-office operations when needed. Key Responsibilities

  • Build meaningful relationships with a select patient panel. 
  • Work closely with our physician in a true collaborative partnership during patient examinations and procedures, ensuring a comfortable and efficient environment. 
  • Implementation and management of Chronic Care Management (CCM), including accurate documentation, patient enrollment, monthly call compliance, and revenue contribution 
  • Provide direct patient care including vital signs, medication administration, injections, immunizations, wound care, and diagnostic testing within scope of practice. 
  • Manage patient records by accurately inputting information into electronic health records (EHR) and maintaining confidentiality in accordance with HIPAA regulations.  
  • Coordinate comprehensive care plans and follow up with patients personally. 
  • Serve as a primary point of contact, responding to patient needs with care and attention. 
  • Apply expanded clinical skills in a thoughtful, patient-centered environment.

Requirements

Required Qualifications

  • ASN or BSN certification 
  • Phlebotomy Certification 
  • Current American Heart Association BLS Certification Card. 
  • Two years of clinical nursing experience, preferably in primary care, concierge medicine, or outpatient practice. 

Preferred Qualifications

  • Experience in a clinical setting, particularly in outpatient practices. 
  • Strong clinical assessment and triage skills. 
  • Familiarity with specific EMR systems (e.g., EPIC, Athena). 
  • Experience with patient education and health promotion.
