Job description

The Heritage Center of Holiday Health Care is seeking a dedicated and reliable Full-Time MDS Nurse to join our team! This individual will serve as one of three members of our MDS team, focusing on completing MDS assessments and participating in resident care conferences to ensure high-quality care for our residents. This is an on-site, in-person position and is not included in the facility's on-call rotation.

We welcome applications from candidates who may not have extensive experience in MDS but demonstrate a strong willingness to learn. Our team is dedicated to providing on-the-job training to help this person succeed.

Key Responsibilities:

  • Collaborate as 1 of the 3 member MDS team
  • Complete and submit MDS assessments for all nursing unit residents
  • Participate in and coordinate resident care conferences to ensure optimal care plans
  • Conduct unit rounds to assess resident function and team interaction
  • Ensure MDS is submitted on time in compliance with State and Federal guidelines
  • Collaborate with the health care team, including physicians, to create an interdisciplinary care plan for each resident
  • Provide education to residents, families, and staff regarding the care provided
  • Monitor the completion of ADL (Activities of Daily Living) assessments
  • Ensure all care plans are aligned with the MDS assessments and RAP guidelines
