Job description

A respected skilled nursing and rehabilitation facility in Rochester, NY, is seeking an experienced and detail-oriented MDS Coordinator (RN) to join our clinical team. We are looking for a strong RN who understands accurate assessment, proper reimbursement, and how to drive quality outcomes through the MDS process.

Key Responsibilities
  • Complete and manage MDS assessments, care plans, and RAI processes according to regulatory timelines.

  • Ensure accuracy of diagnoses, documentation, and coding to support proper PDPM reimbursement.

  • Collaborate with nursing, therapy, dietary, and social work to gather supporting information.

  • Participate in care plan meetings and quality improvement initiatives.

  • Audit records, monitor compliance, and assist with survey readiness.

  • Work closely with the DON and Administrator to drive clinical and financial outcomes.

Qualifications
  • Active RN license in New York (required).

  • Prior MDS/RAI experience in a skilled nursing facility required.

  • Strong understanding of PDPM, ICD-10 coding, and care plan development.
