Job description

) A dynamic skilled nursing facility is searching for experienced candidates for the position of MDS Assessor. Thank you in advance for your interest in Waterview Nursing Care Center. We invite you to become a part of our team and look forward to reviewing your resume.
Our MDS Assessor will work alongside our MDS Coordinator to complete all MDS and Care plans as assigned.
Responsibilities may include are but not limited to:
  • Completes assessments, MDS, and care plans as assigned.
  • Creating and disseminating care plan schedules.
  • Responsible for the entire MDS process.
  • Reviews MDS assessments when there is a change in a resident's condition and/or quarterly.
  • Develops Comprehensive Care Plan with the Interdisciplinary Team.
  • Makes recommendations for charting requirements.
  • Informs staff of any changes regarding resident care and care issues.
  • Monitors "RUG" scores for optimal reimbursement.
  • Monitor and audit records to ensure accuracy and timely
  • Maintain the confidentiality of all residents, staff, and organization information.
  • Other duties may be assigned.

Requirements for RN MDS Assessor:
  • Long Term Care experience
  • Knowledge of PDPM and CMI
  • Well-organized, motivated, and professional
  • Active NYS Registered (RN) License
