MDS Assessor-Nursing Home

Located in Queens, NY

*Salary: $100K - $130K per year (depending on experience)*

Requirements of the MDS Assessor-Nursing Home:

  • Current license as Registered Nurse (RN) in the State of New York
  • Certification in MDS
  • Previous MDS experience gained in Skilled Nursing Facilities
  • Must know MDS 3.0 Experience
  • Knowledge of Medicaid and Medicare
  • Must have long term care experience

Responsibilities of the MDS Assessor-Nursing Home:

  • Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
  • Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
  • Provide clinical support and direct to appropriate resource materials.
