Position Summary
We are seeking a knowledgeable and detail-oriented MDS Assessor to join our team. The MDS Assessor is responsible for completing resident assessments in accordance with federal and state regulations, ensuring accuracy, compliance, and timely submission of the Minimum Data Set (MDS). This role supports quality resident care and appropriate reimbursement.
Key Responsibilities
  • Perform and complete MDS assessments in compliance with CMS and state regulations
  • Accurately document resident conditions and care needs
  • Gather information from medical records, staff, residents, and families
  • Ensure timely submission of MDS data
  • Assist in development and updating of resident care plans
  • Support the interdisciplinary team in care coordination
  • Monitor assessment schedules and deadlines
  • Maintain complete and compliant documentation for audits and surveys
  • Participate in quality improvement initiatives
  • Skilled in Sigma is a plus
Qualifications
