About the Role

We are seeking a passionate, community-focused Advocate to join our MOMobile Managed Care team. This role is ideal for someone who enjoys working directly with families, engaging in the community, and making a meaningful difference in the lives of pregnant women, new parents, and young children.

Key Responsibilities

  • Conduct outreach to identify and engage pregnant women and new parents in need of services
  • Provide case management, including assessment, referrals, follow-up, and ongoing support
  • Assist clients in setting and achieving health and social service goals
  • Ensure timely scheduling of prenatal, postpartum, and well-child appointments
  • Support and facilitate community health education programs and events
  • Represent MCC at community meetings, health fairs, and outreach activities
  • Maintain accurate and confidential case records using database systems
  • Collaborate with internal teams and external partners to support client needs
  • Routinely uses MCC client database and other computer systems, including email, Internet, Microsoft Office, spreadsheets, and customized databases.
  • Support clients in planning for their own health and social service needs by assisting in identifying goals.
  • Refer participants needing more intensive follow-up to appropriate agencies. Ensure timely scheduling of prenatal, postpartum, well-child, and early intervention appointments.
  • Keeps careful records of all contacts made and accurate records of services received by clients.
  • Attends orientation and ongoing training sessions on maternal and child health and welfare, child abuse, housing, nutrition, substance abuse, counseling, parenting, and infant and family support issues.
  • Meets with supervisor regularly to report on and discuss work.
  • Ensures confidentiality of client records, sharing pertinent information only with the client’s approval.
  • Knowledge of and sensitivity to issues facing the community served.
  • Experience in client management procedures for determining eligibility, assessing needs, identifying resources, making referrals, following up, and documenting client interactions.
  • Demonstrated clinical social work skills.
  • Strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, community partners, clients, and members of the community.
  • Effective and accurate documentation of case notes and client data according to established client management procedures.
  • Ability to organize tasks efficiently and follow up and follow through with strong attention to detail.

Qualifications

Required

  • High School Diploma or equivalent
  • Valid Pennsylvania Driver’s License (travel required)
  • Ability to travel in and around the Greater Philadelphia region to conduct home visits and community outreach.
  • Minimum 2 years of experience in community outreach or direct service

Preferred

  • Bachelor’s degree in Social Work, Public Health, or related field
  • Experience working with maternal and child health populations
  • Knowledge of community health systems and social services
  • Bilingual skills (a plus)
